2020-2021 Pre-participation Form

All current participants that had a completed physical on file for the 2019-2020 school year, that physical will carryover for 2020-2021 school year. You are only required to receive a new medical evaluation if you have a medical condition that requires an annual check up. However, you must complete all online paperwork through Sportsware.

Those new to Conroe ISD, incoming 7th graders and new participants in Athletics, Marching Band, Cheerleading, Drill Team, ROTC, and CISD Club Sponsored Athletic Teams will need a physical before participating for the 2020-2021 school year. They also must complete the online paperwork through Sportsware.
